Supplemental Health Care is hiring Physical Therapists with prior OASIS experience to work at a partnering home health care provider in Portage, Michigan. We bring more than 40 years of experience and a reputation for excellence to every therapist we support.

Home Health Physical Therapist Contract Details:

$1,820 â $2,006 per week*8-week contractPaid by the hour â not by the visitPhysical Therapist will have a designated territory of coverage with a set amount of patientsHome Health contracts with SHC require previous experience with OASIS/Medicare Certified Visits*Estimated weekly payment may include both taxable wages and tax-free reimbursements for meals, housing, and incidentals. Requirements:

Graduate of an accredited PT program.

• Licensed as a Physical Therapist in Michigan
• CPR Certified or ability to obtain
• Previous experience with Computer Charting
• 6 months of previous Home Health Care experienceValid U.S. Driverâs License and reliable transportation
  Supplemental Health Care will consider for employment qualified applicants with criminal histories in a manner consistent with the requirements of applicable local, state, or federal law (including San Francisco Ordinance #131192, Los Angeles Municipal Code 189.00, the Los Angeles County Fair Chance Ordinance for Employers and the California Fair Chance Act).
